STUDENT REGISTRATION AND ENROLLMENT

Are you entering or returning to the Lakewood City Schools? 

Student Registration takes place at the Lakewood Board of Education on any business day from 8:00 a.m.- 2:00 p.m. Please call (216) 529-4279 for additional information. The Board of Education is located at 1470 Warren Avenue, Lakewood, Ohio 44107

No appointment is necessary. Be mindful, however, that you may experience some delay at peak times of the day or prior to the start of the school year.

You may access student registration information and appropriate forms in the REGISTRATION INFORMATION and DOCUMENTS sections of this page. You may also obtain a registration packet at the Board of Education.


Who may register a child?

Only legal residents who are the parent or legal guardian of the child may complete the registration process. All certified court orders pertaining to guardianship and custody must be presented at the time of registration. The Lakewood City School District does not grant temporary enrollment (OCR 3313.64 E).

If after your child is enrolled and you move to a new address at any time during your child’s enrollment in the District, it is your responsibility to inform your child’s school principal of your new address. 

Likewise, parent(s)/guardian(s) are responsible for informing school officials of any change(s) to the legal custody or guardianhip of their child(ren).

In the event of any of the changes listed below, parent(s)/guardian(s) shall notify their child(ren)'s school or house principal (LHS only).

NOTE:  Notification shall be made to the Department of Student Services at the Board of Education when school is not in session.

Mandatory Reporting

  • any change of address
  • any change of telephone number(s)
  • any change of legal custody or guardianship
  • any and all current court orders from Domestic Relations, Juvenile, Probate or any other court which has exercised jurisdiction over the custody, guardianship or residence of the child(ren) as stipulated in O.R.C. 3313.672.
     

Kindergarten Information

Ohio Law requires that all children complete a formal kindergarten program. Any child who is 5 years old on or before September 30 of the school year is eligible for kindergarten.

Residency Requirements

The schools of the Lakewood City School District are tuition-free only for those students whose parents/legal guardians are residents of the school district. “Residence” for school purposes means that the custodial parent(s)/legal guardian(s) of the student live within the school district seven days a week.


Required Information for Registration

 AChild’s Information:

  • Certified copy of birth certificate (passport or visa)
  • Social Security card (optional)
  • Immunization record
  • IEP and MFE (for Special Education only); 504 Plan
  • Withdrawal information/report card/transcript from previous school 

 B.  Parent Information:

  • Valid driver’s license / picture I.D.

  • Residency (provide two) :

  1. Current gas/electric bill-portion showing your  name & address
  2. Faxed confirmation of new utility service: (216) 529-4104                    [Attn: Ms. Griveas]
  3. Fully executed lease: Includes landlord contact information and all persons living at this address 
  4. Owner Affidavit completed in full and notarized by landlord/property manager (only when numbers 1, 2, and/or 3 cannot be provided). 
  5. Residency and Custody Affidavit completed in full and notarized by resident.
     

 C.  Custody/Guardianship (provide appropriate certified court journal entry )

  • Divorce decree & shared parenting plan;
  • Guardianship;
  • Placement

D.  Parent Completes:

  • Student Registration Form
  • Release of Records Authorization Form
  • Residency and Custody Affidavit
  • Owner Affidavit (if applicable) 
  • Home Language Survey
  • Emergency Medical Authorization Form

E.  Physician Completes:

  • Physician's Medical Report
  • Medication Administration Form (if student takes medication at school)
